Minimum Privacy Notice

This notice explains how Nilo handles information submitted for the Practice Opportunity Assessment and related written follow-up.

1. Scope

Nilo is currently conducting early-stage practice operations research. The website itself does not directly collect practice operating data. If you choose to complete the assessment, your responses are submitted through the linked Google Form.

2. Information we may receive

The assessment may request business contact information, your role, practice-level operating ranges, current processes, and your views about operational priorities. Please provide only information you are authorized to share.

3. Do not submit patient-identifiable information

Do not submit patient names, phone numbers, email addresses, insurance or member identification numbers, dates of birth, clinical records, treatment details tied to a person, or any other information that can identify a patient. Nilo does not request or intend to receive protected health information through this assessment.

4. How information is used

Information submitted through the assessment is used to:

  • prepare the requested Practice Opportunity Report;
  • clarify or correct practice-level inputs and assumptions;
  • provide written follow-up that you choose to receive; and
  • improve Nilo's research methods using aggregated or de-identified observations where practical.

5. Estimates and report limits

The report may contain modeled estimates based on your answers and conservative assumptions. A modeled estimate is not a confirmed financial loss, audit result, guarantee, or professional legal, accounting, or clinical advice.

6. Service providers

Nilo may use third-party tools, including Google Forms, Google Sheets, Google Docs, email providers, website hosting, and domain services, to carry out the assessment and deliver written materials. These tools are execution services; their use does not change the purpose described above. Their own privacy terms may also apply.

7. Storage, access, and portability

Access to research materials is limited to people supporting the assessment. Nilo intends to maintain research data and analysis in formats that can be exported, backed up, and migrated to another service. No online system can be guaranteed completely secure.

8. Choices and requests

You may ask to correct your business contact information, clarify an assessment response, or request deletion of information that Nilo controls, subject to legitimate recordkeeping needs. A public business contact address will be added before external launch.

9. No certification or legal-review claim

Nilo does not claim that this website or assessment has completed a formal legal review, HIPAA certification, or other regulatory certification. This minimum notice will be reviewed and updated as the service and data practices develop.
